Vendors/Suppliers: If you are not able to order and distribute Vaccination Confirmation Stickers in time for the August 23rd date for the California vaccination mandate, please ensure your vendor representatives are able to show proof of vaccination and/or testing at KP facility entrance and follow all PPE guidelines.
Kaiser Permanente vendors and suppliers are responsible for ensuring their vendor representatives (e.g., employees, subcontractors) who enter a KP facility are fully vaccinated against COVID-19. Fully vaccinated vendor representatives should be provided ONE “vendor confirmation sticker,” per the below requirements, which will provide them entry into a KP facility.

Text: “[Vendor Company’s Initials] 2021” (e.g., CI 2021; UPS 2021)
Please DO NOT use “KP 2021”

Stickers should be placed on badge or another permanent visible location.
