Service Level Agreement
If a supplier is providing software or cloud services to Kaiser Permanente, the supplier shall comply with the Service Level Agreement.
Contingent Worker Access Termination
Vendors providing Contingent Workers (Temporary Contractors, Independent Contractors, Consultants, Offshore Workers, Offsite Service Workers, and On-Premises Service Workers etc.) with access to Kaiser Permanente systems must complete training and follow the operational requirements outlined below to ensure appropriate access and that contingent workers NUIDs are terminated in a timely manner in the event of early or unplanned cessation of work.
Kaiser Permanente HealthConnect® Affiliate Link Access Agreement
Third party entities, including third party providers, must enter into this agreement to access Kaiser Permanente HealthConnect® through Kaiser Permanente’s Affiliate Link program. This agreement provides the terms and conditions under which Kaiser Permanente will grant such access to certain third parties.
Vendor Financial Risk Assessment Program Requirements
Supplier will comply with Kaiser Permanente’s Vendor Financial Risk Assessment Program Requirements. Supplier is responsible for responding to a request for financial information within ten business days of receipt of request.
Vendor Portal Registration Program Requirements
Supplier will comply with Kaiser Permanente’s Vendor Portal Registration Program Requirements. Supplier is responsible for entering and maintaining the accuracy of their company information residing on the Vendor Portal.
Medical Device Security Requirements (formerly Edge Security Requirements)
Suppliers must comply with Kaiser Permanente's Medical Device Security Requirements if Supplier will be supplying or supporting any device that falls into the governance of Kaiser Permanente’s medical device cybersecurity program, to include but not limited to medical devices (including laboratory and imaging), pharmacy devices, clinical research technologies, and supporting devices.
Mutual Non-Disclosure Terms and Conditions
These downloadable Kaiser Permanente® Mutual Non-Disclosure Terms and Conditions (“NDA”) apply to transactions or interactions between a supplier/contractor and a Kaiser Permanente entity which involve the disclosure of confidential or proprietary information, unless the parties have agreed in writing to use other terms and conditions. The NDA also applies to written agreements which incorporate by reference the NDA, including, without limitation, a Mutual Non-Disclosure Letter Agreement.

Kaiser Permanente uses artificial intelligence (AI) to improve high-quality and affordable care with consideration for its members, clinicians, and employees and aims for the AI tools that it uses to be safe, reliable, equitable, and trustworthy. To meet this objective, Kaiser Permanente has created these Artificial Intelligence Requirements to set forth the AI requirements for vendors, contractors and suppliers that will support its responsible AI approach.
Insurance Requirements
Kaiser Permanente requires suppliers to procure and maintain insurance coverage. These Insurance Requirements contain the minimum insurance coverage levels.
